WAIVERED SERVICES

While we offer variety of programs, our hope is to create a supportive and nurturing environment, thus enabling each resident to be more carefree, while having an opportunity to be helped with changing physical needs, social abilities, and to enjoy a fulfilling life.

24-Hour Emergency Assistance

On-call counseling and problem solving and/or immediate response for assistance at a person’s home due to a health/personal emergency.

Night Supervision

Overnight assistance and monitoring provided by an awake staff in the person’s home.​

Respite Care

BI, CAC, CADI and DD Waiver plans, Respite: Short-term care services provided due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.​

Individualized home supports with family training

We offer household chores includes washing dishes, vacuuming, dusting, changing/ making bed, running errands helping with grocery shopping freshly preparing meals, clean clothes, laundry and linens and helping with organization around the home, Arranging for transportation.

Individualized home supports without training

Individualized home supports without training is non-medical social support services for people with disabilities or older adults who need supervision, care, or assistance, Laundry, Light housekeeping tasks, Meal preparation, Shopping, Supervision, Socialization.

Non-medical care, supervision and assistance provided in the home of the person or in the community to achieve increased independence, productivity and inclusion in the community. Living Skills Training Services​

Individualized home supports with training (IHS):

Services for people who live in their own home or family home that provide support and/or training in the community living service categories listed in the covered services section. There are three types of IHS:

  • Without training
  • With training
  • With family training

All types of IHS can be provided in the person’s own home, family’s home or in community spaces used by the general public, and either in person or remotely.

Household Management

This category may include:

  • 1. Cueing, guidance, supervision, training or instructional support to complete routine household care and maintenance.
  • 2. Household safety knowledge and skills.
  • 3. Tenancy support and advocacy.
  • 4. Training, assistance, support and/or guidance with:
  • Budgeting and assistance to manage money.
  • Cooking, meal-planning and nutrition.
  • Healthy lifestyle skills and practices.
  • Household chores, including minor household maintenance activities.
    Note: The person is responsible for the cost of the maintenance replacement items or products.
  • Personal-needs purchasing.

Adaptive Skills

This category may include:

  • Crisis prevention skills.
  • Implementation of positive support strategies.
  • Problem-solving.
  • Sensory/motor development involved in acquiring functional skills.
  • Support strategies for self-sufficiency.
  • Support and training to increase positive behavior, resulting in reduction or elimination of challenging behavior.
